YOUNG DEVON |
COMMUNITY CONNECTORS: GP SURGERIES ACROSS THE CITY CAN INTRODUCE A PERSON THEY BELIEVE WOULD BENEFIT FROM BEING BETTER CONNECTED TO THEIR COMMUNITY TO A COMMUNITY CONNECTOR. CONNECTORS TAKE TIME TO LISTEN TO INDIVIDUALS, AND TO BUILD A HOLISTIC PICTURE OF WHAT A GOOD LIFE MEANS TO THEM. THEY THEN WORK TOGETHER TO DECIDE HOW TO MOVE TOWARDS THEIR GOALS, DRAWING ON THE FIVE WAYS TO WELLBEING, TO DEVELOP AN INDIVIDUAL AND SUSTAINABLE ACTION PLAN. This grant application is to fund 75 (2 full time equivalents) hours of Community Connector time per week as part of the Wellbeing Exeter partnership. The Community Connectors employed by Young Devon will work with individuals between the ages of 11 and 18. Individuals aged 11 or 12 will require parental consent to participate in Wellbeing Exeter. |

The Community Connectors are employed and hosted by CoLab Exeter, one of Wellbeing Exeter’s existing delivery partners and are line-managed and supervised by the Community Connections Manager, part of the Wellbeing Exeter Coordination team based at CoLab. AGE UK EXETER |
Age UK Exeter is one of a group of voluntary / community sector organisations providing and hosting the Community Connector role for Wellbeing Exeter. Age UK Exeter will employ two Community Connectors – Culture (CCC) posts (0.6FTEs x 2) in close partnership with RAMM. RAMM will offer day-to-day support and supervision for the two employees and office space as required. This is a new partnership and our aim is for the two CCCs to work together and individually on projects to engage Exeter residents aged over 50 with the range of cultural assets in the city, which includes the museum, arts venues, historic building, libraries and archives, cultural events, festivals and other creative/ cultural opportunities. Our work is led by the needs and interests of the individuals we work with, so the CCCs will need to be flexible and responsive when working with older residents. This is an exciting, creative and flexible role. Individuals aged over 50 will be referred to Community Connectors who meet with the person and talk with them about their life, interests and the barriers they face. It is the Community Connector’s role to listen to and understand the person’s circumstances, work alongside them to build on their interests and to find out what cultural opportunities are out there that that might enrich their life while overcoming barriers. For example, it might be outreach work with museum handling objects as part of a memory café, joining a local craft group, visiting the museum or cultural venue as a safe and welcoming destination for a trip out of the house, or contributing to a community exhibition. The goal of Community Connecting is always for the work to have longevity and be something that can continue long after the work with a Connector has finished. |

CoLab Exeter is one of a group of voluntary/community sector organisations providing and hosting the Community Connector role for Wellbeing Exeter. Currently this involves staff from GP practices and other healthcare professionals, making referrals to the Wellbeing Exeter Single Point of Referral for people who they think could benefit from the support available in the community and voluntary sector. The Wellbeing Exeter programme is available to adults aged 18 and over, Young People (11+) and Families. This role will be focused on working alongside those aged 30 years + across the city. Referrals are allocated to Community Connectors who meet with the person and talk with them about their life, interests and the barriers they face. It is the Community Connector’s role to listen to and understand the person’s circumstances, work alongside them to build on their interests and to find out what’s out there that might enrich their life while overcoming barriers. For example, it might be about introducing people to social or exercise groups or activities, or about helping people to access information, services and advice. The goal of Community Connecting is always for the work to have longevity and be something that can continue long after the work with a Connector has finished. |

CORNWALL COMMUNITY FOUNDATION |
The Chalk Effect Climbing Project aspires to be a free indoor climbing activity and a mental health and well-being service that embraces a movement of change and how we manage our well-being and mental health. Chalk instructs all aspects of Indoor Rock Climbing to empower young people. The young people that are referred come from a multitude of backgrounds. Case study: A young lady aged 15 came to the project but she didn’t want to be seen, she stood at the back of the group, hood up head down. The group put their harnesses on and waiting in turn for their chance to climb, then it came to Marnie, she had shoes on that were not hers, hair tied back quietly willing herself to go. She took off her hoody revealing scars all the way up to her shoulders but she was here and safe from whatever demons lay inside her mind. Clipped into the rope and climbing, she came alive. From that session she continued to complete climbs one after another. She has now enrolled on the project and starting to give back to the new comer climbers, she is communicating with others, becoming more vocal in and out of the project, her confidence is improving weekly as well as her physical climbing. |
