Support for people in Nottingham with mental health needs to address their financial, housing and health concerns enabling them to live independently. Support workers will visit beneficiaries in their own home for an initial review and draw up an action plan with one-to-one support on a weekly or fortnightly basis as appropriate plus advice and guidance to work through problems. There will also be a facilitated monthly social group and a trained volunteer befriender to help them establish independence.
