East Cheshire Hospice provides expert palliative care in the community with their Hospice @Home service and in their In Patient Unit free of charge to those adults facing end-of-life in East Cheshire. The Workforce Wellbeing COVID-19 Recovery project will enable clinical staff to continue to care for those impacted by the COVID-19 pandemic struggling with isolation and loneliness. The grant will fund a 12 month COVID-19 impact recovery counselling and training programme to build resilience in the clinical team. During and post the pandemic the Hospice has lost nearly 2000 clinical working hours directly attributed to the impact of COVID-19, the majority within the Hospice @Home clinical team. The community nursing team has been a lifeline for those who have been isolated due to Government rules and felt at their most vulnerable facing life limiting illness. East Cheshire Hospice aim always to put the patient at the centre of everything they do.

1. East Cheshire Hospice care for those individuals and families living with a Life limiting illness. Our expert and experienced Palliative care nurses support our community in our Hospice and in the community. 2. The impact of Coronavirus has made face to face communication and information sharing difficult, therefore it is essential to use technology to help the continuity of communication between our Hospice @Home nurses and other healthcare professionals. 3. The grant will help fund ipads and technology to be used by our Hospice @Home nurses in the community and enable clear and accurate information sharing to help support those that need us most. 4. Using the technology our Hospice @Home nurses will have constant access to the EMIS software which contains all current and up to date medical notes, information is recorded in real time safeguarding the patient. The ipads will also allow our nurses conferencing facilities to consult with other healthcare professional therefore improving the care for the patient and their families.

1. East Cheshire Hospice Dementia Carer Well-Being Service, supporting carers of people with dementia. 2. The programme is designed to help people who are supporting a person with dementia in their own home. 3. The grant is vital in helping to fund the 8 week structured support programme, 3 time a year. Each session costs £966 to run, the £4,000 requested will enable 24 carers and their loved ones suffering from dementia to benefit from the programme over 4 Well-being days. 4. The aim is to provide a safe, relaxed environment whereby carers can share problems, concerns, learn about planning for the future, managing finances, relaxation and complimentary therapies, dietary and nutritional advise. The carer is more able to cope and gives much needed rest bite.

1. East Cheshire Hospice Childhood Bereavement Service, supporting young people to cope with death and dying 2. Our aim is to be the 'go to place' for all referrals of childhood bereavement in East Cheshire for anyone aged 18 or under having mental or physical difficulty dealing with the loss of a person(s) close to them 3. The grant is vital for the continuation of service which delivers 1:1 counselling, peer support groups, information & advice for relatives and healthcare professionals and running training courses for teachers and youth workers as well as awareness raising activity. 4. The grant is essential in enable our childhood bereavement service to improve the mental health of 1130 children and give their families and/or carers the tools to support them through their bereavement journey in East Cheshire every year.

East Cheshire Hospice provides essential end of life care and support to adults with a wide range of diagnoses. This project is a new @Home service which aims to bring high quality care and expertise to the patient where they are at home. The project offers expert end of life nursing care in homes and care settings during out of hours, evenings, nights and weekends including bank holidays 365 days of the year. The new service will enable the Hospice to care for an additional 250 patients per year without the cost of building a new Hospice. The grant funding provides 'seed funding' which will help to launch the service and keep it running in the first five years, enabling the charity the time and space to develop other income streams to ensure that care is delivered for generations to come. The grant will pay for building alterations to allow 24 hour secure door access for the Hospice @Home team and set up costs such as branded uniforms, nurses mobile equipment bags, communication and awareness raising materials.

East Cheshire Hospice identified a need for the Living Well with Motor Neurone Disease (MND) project - a wide ranging support service for people with Motor Neurone Disease and their carers (MND is a progressive disease with symptoms affecting walking, talking, eating and breathing). With limited life expectancy post-diagnosis, it is critical that each person living with MND be supported through the project to live every moment as well as possible. The grant will fund staffing and running costs.

The aim of this new project is to provide targeted pre and post bereavement support for children who have lost or are about to lose a significant adult in their lives. The service will initially be targeted at families who have a loved one residing at the Hospice, and then in time will be widened out to referrals from agencies such as the Macmillan Nurses.
