The funding will help develop robust plans for collaboration with other organisations, new initiatives focussing on more sustainable hospital discharges for older people. We need to look at how ways of working, track record and flexibility can complement the work of the local care partnerships to the betterment of local older people. The grant will afford us breathing space to focus on alternative sources of more longer-term funding and to develop plans to bring the whole organisation with us on this journey.
