This will facilitate recovery and reduce isolation, thus promoting wellbeing, preventing hospital admissions and supporting those who have been discharged. 
27/09/2022 £22,500 TAMESIDE OLDHAM AND GLOSSOP MIND  To build on the current VCSE partnership pilot (with Age UK Oldham and Positive Steps), a DTOC service called ‘Steps to Home, for patients in Mental Health and Community Care at the Royal Oldham Hospital who are ready to be discharged but need additional support to do so. 
27/09/2022 £9,880 £747,032 WAI YIN SOCIETY  To provide a culturally appropriate service to people from the Chinese community requiring support upon discharge from hospital, based on positive evidences from the “Winter Wonderland Hospital Discharge Project”, funded from January to March 2022. Service includes befriending scheme, advice and information support, and meal delivery.
