SEBASTIAN'S ACTION TRUST 

We support families of children with life-limiting conditions, aged 0-19, from point of diagnosis, through treatment and beyond, by listening and responding to their unmet need.Our holistic model of care provides fully accessible respite breaks for families living in the UK. We also provide outreach support services to families living in Buckinghamshire, Berkshire, Hamps, Surrey and Oxfordshire.
